أنا جديده هنا احتاج مساعدة منكم لو سمحتوا

اترجم كاتب طبي عن سرطان الثدي وفيه بعض المصطلحات و الاختصارات لم اجد المقابل العربي لها

وتعبت وانا ابحث ولم اجدها ولا وقت كافي لدي اريد رد عاجلا من اي احد يستطيع ان يفيدني بأي شيء و لن انسى جميل لكم..

سأضع الكلمات هنا مرفقه بالنص، لا اريد ترجمه للنص فقط للكلمات اللملونه بالأحمر

هي مواد ذات طبيعة بروتينية موجودة في جسمنا وتفرز بشكل طبيعي ولها نسبة تركيز معينة في وضع الجسم الطبيعي

وازدياد نسبة تركيزها إلى نسب معينة يدل على وجود سرطانات ,ولذلك يطلب تحليلها لكي نعرف نسبتها هل هي طبيعية أم مرتفعة إذا CA موجودة في الشخص الطبيعي والشخص المصاب بسرطان يعني موجودة بجسمي وجسمك ولكن نسبتها هي التي تحدد وضع المريض أي:

اذا كانت النسبة طبيعية لامشكلة في ذلك والمريض سليم

اذا كانت النسبة مرتفعة فهذه اشارة خطرة تفيد بوجود أورام سرطانية

وصلت الفكرة..ولا لأ

بالنسبة ل br
فتشير إلى كلمة breast

بس بإعتبار أني اختبار CA 27-29 يستخدم بكثرة للكشف عن سرطان الثدي ففي بعض المراجع صارت تستبدل كلمة CA بكلمة BR اللي مأخودة من كلمة BREAST

فالمقصود في هذا السياق هنا هو دراسة الخزعة من الناحية النسيجية ,, و التشاور مع الأخصائي الذي يدرسها


النقطة الثانية و هي مفهوم الرأي الثاني ,, و هي أنه كثيراً ما يأتينا مريض كان يتلقى العلاج في مركز آخر أو بلد مجاور
و هنا نفضل أن يحضر لنا الشرائح النسيجية ليفحصها أخصائي التشريح المرضي النسيجي الموجود في مركزنا( مستشفى - مركز أبحاث- جامعة -.........) و هذا يتيح لنا فرصة الالتقاء مع هذا الأخصائي و مشاورته

بالإضافة لذلك فإن معظم المراكز تفضل عرض الخزعات على مستشفى آخر أو أخصائي آخر بقصد تأكيد العلاج



بالنسبة لموضوع المستقبلات ,,, هناك أنواع من المستقبلات على سطح الخلية السرطانية:
1 - المستقبلات الهرمونية ,,, و هي مستقبلات تتأثر بالأستروجين أو البروجيستيرون ,, حيث تعزز هذه الهرمونات من نمو الورم
2 - المستقبلات الأخرى : عديدة ,أهمها HER-2-neu و هي اختصار لعبارة Human Epithelial Receptor 2 و معناها المستقبلات البطانية الإنسانية(البشرية) و لم أسمع أن أحداً قام بترجمتها ,, بل نذكرها و كأنها اسم علم(هير-تو- نيو)


بالمناسبة هناك مستقبلات هير-1 في سرطان القصبات


و لنا عودة إن شاء الله

عادة بعد أخذ الخزعة يتم إرسال العينة لأخصائي التشريح المرضي لدراسة أشكال و أحجام و توزع الخلايا ,, و هذا يشخص عادة المرض


بعدها يمكن أن نجري تلوينات خاصة لبروتينات خلوية معينة أو مستقبلات ,, و هذا يدعى الدراسة الكيماوية المناعية,,
Immunohistochemistry (IHC) و هذا يعطينا فكرة أدق عن نوعية الخلايا و صفاتها ,, مثل المستقبلات الهرمونية و مستقبلات هير 2 نيو



و كذلك يمكن إجراء دراسة لبنية الحمض النووي في الخلايا و تحري طفرات معينة مثل BRCA و هذا يدعى Cytgenetics


الدراسة بالتلوينات المناعية غير متوفرة في كل المراكز ,,,,
و هي تؤثر في طريقة العلاج ,, فاليوم لدينا علاج هرموني نستخدمه عند وجود المستقبلات الهرمونية
و لدينا علاج حيوي و هو أجسام مضادة وحيدة النسيلة Monoclonal Antibodies خاصة بالمستقبلات (هير2 نيو) ,, و هي تستخدم عند إيجابية المستقبلات
هذا بالإضافة للعلاج الكيماوي




هذه المعلومات المبسطة هي ملخص ما يجري في مراكز الأورام ,,, و معرفتها تسهل فهم هذا النوع من المقالات

Bisphonsphonates are antiresorptive agents drugs that interfere with the normal cycles of bone building and destruction (resorption) that permit the body to reshape the skeleton throughout life. Because of this ability, they are widely used to treat osteoporosis. Studies have shown that these drugs are also able to prevent or delay the spread of breast cancer to bone. In the US, pamidronate (Aredia) and zoledronic acid (Zometa) are the most widely used drugs of this group for the treatment of breast cancer. Abstract
Dietary intake of tomatoes and tomato products containing lycopene has been shown to be associated with a decreased risk of chronic
diseases such as cancer and cardiovascular disease in several recent studies. Serum and tissue lycopene levels have also been inversely related
to the risk of chronic disease. While the antioxidant properties of lycopene are thought to be primarily responsible for its beneficial effects,
evidence is accumulating to suggest that other mechanisms, such as modulation of intercellular gap junction communication, hormonal and
immune systems, and metabolic pathways, may also be involved. Lycopene inhibited the activity of an essential enzyme involved in
cholesterol synthesis in an in vitro study and a small clinical study, suggesting a hypocholesterolemic effect.

1. Introduction
People who live in and around the Mediterranean present
with a lowered risk of several important chronic diseases,
including coronary heart disease and a number of types of
cancer (breast, colon, prostate). This lowered risk appears to
be related to a traditional diet consisting largely of fruits and
vegetables in general, and of cooked tomatoes in particular,
plus considerable amounts of olive oil. These findings have
lead to public health recommendations to consume more
vegetables, especially cooked tomatoes with olive oil [1].
In a prospective study by Fawzi et al. that examined the
relationship between the consumption of tomatoes and
morbidity and mortality among 28,753 children aged 6–
60 months, it was found that an intake of tomatoes for 2–3
days (compared with 0 days) was associated with a
significant reduction in mortality (48%) and with a reduced
risk of death associated with diarrhea. Intake of tomatoes
was also inversely and significantly associated with
respiratory infections [2].
Lycopene, a carotenoid without provitamin-A activity, is
present in many fruits and vegetables; yet, it is tomatoes and
processed tomato products that constitute the major source
of lycopene in the North American diet. Several recent
studies have shown that a dietary intake of tomatoes and
tomato products is associated with a decreased risk ofchronic diseases such as cancer and cardiovascular disease
[3]. Serum and tissue lycopene levels have also been
inversely related with the risk of chronic disease [3]. While
the antioxidant properties of lycopene are thought to be
primarily responsible for its beneficial effects, evidence is
accumulating to suggest that other mechanisms, such as
modulation of intercellular gap junction communication,
hormonal and immune systems, and metabolic pathways,
may also be involved [3].
Lycopene functions as a very potent antioxidant, and this
is clearly an important mechanism of action of lycopene. It
can trap singlet oxygen and reduce mutagenesis. In
physiological concentrations, it can inhibit human cancer
cell growth by interfering with growth factor receptor
signaling and cell cycle progression, specifically in prostate
cancer cells, without evidence of toxic effects or cell
apoptosis. Studies of human and animal cells have identified
a gene, connexin 43, whose expression is upregulated by
lycopene and that allows direct intercellular gap junctional
communication (GJC). GJC is deficient in many human
tumors, and its restoration or upregulation is associated with
decreased proliferation [4].


2. Tomatoes and coronary heart disease
Oxidative stress induced by reactive oxygen species
(ROS) is also considered to play an important role in the
etiology of coronary heart disease. Oxidation of the
circulating low-density lipoprotein LDL is thought to play
a key role in the pathogenesis of atherosclerosis and
coronary heart disease. A recent study has demonstrated
that healthy human subjects who ingested lycopene in the
form of tomato juice or tomato sauce for 1 week had a
significantly lower level of oxidized LDL than controls [5].
Four other human trials have also demonstrated the
antioxidant effects of lycopene [5]. Lycopene was shown
to inhibit the activity of an essential enzyme involved in
cholesterol synthesis in both an in vitro study [5] and a
small clinical study [5], suggesting a hypocholesterolemic
effect. Other possible mechanisms include enhanced LDL
degradation, LDL particle size and composition, plaque
rupture, and altered endothelial functions. Recent epidemiological
studies have also shown an inverse relationship
between tissue and serum levels of lycopene and mortality
from coronary heart disease, cerebrovascular disease, and
myocardial infarction [5]. However, the most impressive
population-based evidence comes from a multi-center, casecontrol
study where subjects from ten European countries
were evaluated with regard to a relationship between
antioxidant status and acute myocardial infarction. After
adjusting for a range of dietary variables, only lycopene
levels were found to be protective [5].
Some 725 middle-aged men, free of coronary heart
disease and stroke, took part in the Kuopio Ischemic Heart
Disease Risk Factor (KIHD) study. Men in the lowest
quartile of serum levels of lycopene had a 3.3 fold increased
risk of an acute coronary event or a stroke as compared to
the other groups [6].
In a second study [6], the association between plasma
lycopene concentration and intima-media thickness of the
common carotid artery wall (CCA-IMT) was examined.
This study was part of the Antioxidant Supplementation in
the Atherosclerosis Prevention (ASAP) study, in which 520
asymptomatic men and women participated. It was found
that low plasma levels of lycopene were associated with an
18% increase in IMT in men compared with men whose
plasma levels were higher than the median. In women, the
difference did not remain significant after the adjustment
[6].


3. Tomatoes and cancer
Tomato consumption showed a consistent inverse relation
with the risk of digestive tract neoplasms in an
integrated series of studies conducted in Italy in the 1980s
[7]. Between 1992 and 1999, a case-control study was
conducted in different areas of Italy on patients below age
80 years of age with different kinds of cancers, i.e., oral
cavity and pharyngeal, esophageal, colorectal, breast, and
ovarian [7]. The control group consisted of 5000 patients,
also below the age of 80, with acute, non-neoplastic
diseases. The multivariate relative risk of oral, pharyngeal
and esophageal cancer decreased with subsequent levels of
lycopene intake (0.7 for oral and pharyngeal cancers and 0.7
for esophageal cancer, both with borderline statistical
significance). In colorectal, breast, and ovarian cancers,
tomato intake was inversely and significantly correlated
with cancer risk (RR=0.8) [7].
Thirty-two patients with localized prostate adenocarcinoma
consumed tomato sauce-based pasta dishes for 3
weeks (30 mg lycopene daily) before their scheduled radical
prostatectomy. Prostate tissue was obtained as a biopsy at
baseline and as a resected tissue at the time of the
prostatectomy. Serum and prostate lycopene, serum prostate-
specific antigen (PSA) concentrations, and leukocyte
DNA 8-OH deoxyguanosine/deoxyguanosine (8OhdG)
were measured at baseline and at the end of the intervention.
Mean serum PSA concentrations decreased by 17.5%
(P <0.002) and leukocyte 8OhdG decreased by 21.3%
( P <0.005) after tomato sauce consumption. Resected
tissues from tomato sauce-supplemented patients had
28.3% lower prostate 8OhdG compared with the control
group (P <0.03). Cancer cell 8OhdG staining in the resected
prostate sections was reduced by 40.5% in mean nuclear
density (P <0.005) and by 36.4% in mean area (P <0.018).
The apoptotic index was higher in hyperplastic and neoplastic
cells in the resected tissue after supplementation.
These data indicate a significant uptake of lycopene into
prostate tissue and a reduction in DNA damage in both
leukocyte and prostate tissue [8].
A clinical study investigated the biological and clinical
effects of lycopene supplementation among patients with
localized prostate cancer [9]. Thirty-six patients with newly
diagnosed prostate cancer were randomly assigned to
receive tomato extract containing 30 mg of lycopene (15
patients) or no supplementation (11 patients) for 3 weeks
before radical prostatectomy. Subjects in the intervention
group had smaller tumors (45% vs. 80%), less involvement
of surgical margins and/or extraprostate tissues with cancer
(73% vs. 18%), and less diffuse involvement of the prostate
by high-grade prostatic intraepithelial neoplasia (33% vs.
0%) compared with subjects in the control group. Mean
PSA levels were lower in the intervention group than in the
control group [9].


4. Conclusion
The consumption of tomatoes has been demonstrated
clinically and in in vitro studies to have beneficial,
protective effects with regard to coronary artery disease
and several neoplasms. Still, the mechanism of action is not
totally clear. Double-blind, placebo-controlled studies
should be carried out in order to clarify this interesting
possible association between diet and health, especially in
the prevention of chronic diseases and cancer.

وقد تبين أن مستويات الليكوبين النسيجي والمصلي متناسبة عكسياً مع خطر حدوث الداء المزمن.

ويعتبر الليكوبين مضاد أكسدة قوي، وهذا واضح جداً في آلية عمله. حيث يمكنه اقتناص الأوكسجين الوليد، كما ينقص معدل الطفرات.

وهو بتراكيزه الطبيعية يثبط نمو الخلايا السرطانية البشرية، بتداخله مع مستقبلات إشارة عوامل النمو، ودورة تطور الخلية، وخصوصاً في سرطان الموثة (البروستات)، بدون إحداث تأثيرات سمية أو موت خلوي.

وبينت الدراسات على الخلايا الحيوانية وجود جين مرتبط على الوصيلة 43 والتي يتعلق نشاطها (أو تعبيرها) بإطلاق upregulation لليكوبين حيث يسمح بالاتصالات العقدية الفجوية بين الخلوية المباشرة gap junctional communication (GJC)

وهي متواجدة في كثير من الأورام البشرية .
كما أن إعادة التخزين والإطلاق مرتبط بإنقاص نسبة التكاثر الخلوي.

Odds ratios (OR) : ربما نترجمها بعبارة : نسبة الاحتمالات ( أحد الزملاء ذكر لي أنه يستخدم مصطلح : معدل الحصيل ,) ,, حيث يقومون بدراسة
1 - نسبة المرضى الذين تعرضوا لعامل معين : و أصيبوا بالسرطان ,,, و نسبة الذين لم يصابوا
2 - ثم يدرسون نسبة من لم يتعرض لهذا العامل و أصيب بالمرض ,, و نسبة من لم يتعرض و لم يصب

و بعدها تحسب التناسبات بينها بقانون خاص لمعرفة إذا كان هذا العامل يترافق مع الإصابة بالسرطان أو لا !!

أرجو أن تكون الفكرة قد وصلت ,, فهذا يساعد في ترجمة بقية المصطلحات المطلوبة

فلعلك لا تستطيع أن تقيم أي الشخصين أطول إذا كان كل منهما يقف على درجة مختلفة على السلم ,, فلتجنب الخطأ في وجود عوامل مختلفة في الإحصاء نستخدم unconditional


أما كلمة regression فهي في الإحصاء لقياس العلاقة بين المتوسط الحسابي لمتغير معين و علاقته بمتغير آخر


و يمكن تجاوز العبارة بالكامل بذكر جملة بسيطة مثل:
التقييم قد تم باستخدام نماذج إحصائية متعددة لتجنب الخطأ قدر الإمكان ,, تم تعديلها من أجل العمر و الجنس ,,,,,,

الضغط الأسمولي أو الحلولي يعمل على سحب الماء من خارج الشعيرات أو الخلايا إلى الداخل ,, وارتفاعه يزيد دخول الماء من الوسط الآخر إلى الوسط عالي الحلولية أو عالي الأسمولية

أما الضغط السكوني للسوائل(هيدروستاتايك) فهو الضغط الفيزيائي العادي للسوائل بالمفهوم الفيزيائي البسيط ,, حيثما كلما زاد هذا الضغط فسيسهم في دفع السوائل وإخراجها إلى الوسط الآخر

Hydatidiform Mole
Definition
A hydatidiform mole is a relatively rare condition in which tissue around a fertilized egg that normally would have developed into the placenta instead develops as an abnormal cluster of cells. (This is also called a molar pregnancy.) This grapelike mass forms inside of the uterus after fertilization instead of a normal embryo. A hydatidiform mole triggers a positive pregnancy ([Only Registered Users Can See Links]) test and in some cases can become cancerous.
Description
A hydatidiform mole ("hydatid" means "drop of water" and "mole" means "spot") occurs in about 1 out of every 1,500 (1/1,500) pregnancies in the United States. In some parts of Asia, however, the incidence may be as high as 1 in 200 (1/200). Molar pregnancies are most likely to occur in younger and older women (especially over age 45) than in those between ages 20-40. About 1-2% of the time a woman who has had a molar pregnancy will have a second one.
A molar pregnancy occurs when cells of the chorionic villi (tiny projections that attach the placenta to the lining of the uterus) don't develop correctly. Instead, they turn into watery clusters that can't support a growing baby. A partial molar pregnancy includes an abnormal embryo (a fertilized egg that has begun to grow) that does not survive. In a compete molar pregnancy there is a small cluster of clear blisters or pouches that don't contain an embryo.
If not removed, about 15% of moles ([Only Registered Users Can See Links]) can become cancerous. They burrow into the wall of the uterus and cause serious bleeding. Another 5% will develop into fast-growing cancers called choriocarcinomas. Some of these tumors spread very quickly outside the uterus in other parts of the body. Fortunately, cancer ([Only Registered Users Can See Links]) developing from these moles is rare and highly curable.
Causes and symptoms
The cause of hydatidiform mole is unclear; some experts believe it is caused by problems with the chromosomes (the structures inside cells that contain genetic information) in either the egg or sperm, or both. It may be associated with poor nutrition ([Only Registered Users Can See Links]), or a problem with the ovaries or the uterus. A mole sometimes can develop from placental tissue that is left behind in the uterus after a miscarriage ([Only Registered Users Can See Links]) or childbirth ([Only Registered Users Can See Links]).
Women with a hydatidiform mole will have a positive pregnancy test and often believe they have a normal pregnancy for the first three or four months. However, in these cases the uterus will grow abnormally fast. By the end of the third month, if not earlier, the woman will experience vaginal bleeding ranging from scant spotting to excessive bleeding. She may have hyperthyroidism ([Only Registered Users Can See Links]) (overproduction of thyroid hormones ([Only Registered Users Can See Links]) causing symptoms such as weight loss, increased appetite, and intolerance to heat). Sometimes, the grapelike cluster of cells itself will be shed with the blood during this time. Other symptoms may include severe nausea and vomiting ([Only Registered Users Can See Links]) and high blood pressure. As the pregnancy progresses, the fetus will not move and there will be no fetal heartbeat.
Diagnosis
The physician may not suspect a molar pregnancy until after the third month or later, when the absence of a fetal heartbeat together with bleeding and severe nausea and vomiting indicates something is amiss.
First, the physician will examine the woman's abdomen, feeling for any strange lumps or abnormalities in the uterus. A tubal pregnancy, which can be life threatening if not treated, will be ruled out. Then the physician will check the levels of human chorionic gonadotropin (hCG), a hormone that is normally produced by a placenta or a mole. Abnormally high levels of hCG together with the symptoms of vaginal bleeding, lack of fetal heartbeat, and an unusually large uterus all indicate a molar pregnancy. An ultrasound of the uterus to make sure there is no living fetus will confirm the diagnosis.
Treatment
It is extremely important to make sure that all of the mole is removed from the uterus, since it is possible that the tissue is potentially cancerous. Often, the tissue is naturally expelled by the fourth month of pregnancy. In some instances, the physician will give the woman a drug called oxytocin to trigger the release of the mole that is not spontaneously aborted.
If this does not happen, however, a vacuum aspiration can be performed to remove the mole. In a procedure similar to a dilatation and curettage ([Only Registered Users Can See Links]) (D & C), a woman is given an anesthetic (to deaden feeling during the procedure), her cervix (the structure at the bottom of the uterus) is dilated and the contents of the uterus is gently suctioned out. After the mole has been mostly removed, gentle scraping of the uterus lining is usually performed.
If the woman is older and does not want any more children, the uterus can be surgically removed (hysterectomy ([Only Registered Users Can See Links])) instead of a vacuum aspiration because of the higher risk of cancerous moles in this age group.
Because of the cancer risk, the physician will continue to monitor the patient for at least two months after the end of a molar pregnancy. Since invasive disease is usually signaled by high levels of hCG that don't go down after the pregnancy has ended, the woman's hCG levels will be checked every two weeks. If the levels don't return to normal by that time, the mole may have become cancerous.
If the hCG level is normal, the woman's hCG will be tested each month for six months, and then every two months for a year.
If the mole has become cancerous, treatment includes removal of the cancerous issue and chemotherapy ([Only Registered Users Can See Links]). If the cancer has spread to other parts of the body, radiation will be added. Specific treatment depends on how advanced the cancer is.
Women should make sure not to become pregnant within a year after hCG levels have returned to normal. If a woman were to become pregnant sooner than that, it would be difficult to tell whether the resulting high levels of hCG were caused by the pregnancy or a cancer from the mole.
Prognosis
A woman with a molar pregnancy often goes through the same emotions and sense of loss as does a woman who has a miscarriage. Most of the time, she truly believed she was pregnant and now has suffered a loss of the baby she thought she was carrying. In addition, there is the added worry that the tissue left behind could become cancerous.
In the unlikely case that the mole is cancerous the cure rate is almost 100%. As long as the uterus was not removed, it would still be possible to have a child at a later time.

وهو مرض إنتاني ينجم عن المتحول الأميبي أو أنواع من البكتريا ,, تصيب جدار الكولون ,, ويؤدي لحدوث براز يحتوي الدم والمخاط


أما كلمة زحير Tenesmus فهي عرض يعاني منه المريض بسبب إصابة المستقيم , وحدوث تخريش مستمر للأعصاب

Petechiae: (الحبرات): بقع حمراء دائرية صغيرة (أقل من 3 مم) , آلية حدوثها هي النزف من الشعيرات داخل الجلد (نزف داخل الأدمة)

ecchymosis: (الكدمات) : نزف في النسج تحت الجلد ,, لونها أزرق ,, قطرها أكبر من الحبرات ,, لونها أزرق بنفسجي
(والمصطلح العامي لها الشائع جداًُ في الاستخدام هو bruise)

وما المقصود بال warm antibody وال cold antibody ..

warm antibody : الأضداد الحارة ,, أي التي تحدث ظاهرة التراص في درجة الحرارة العالية (أكثر من 37° C)
